SnapCare is seeking a dedicated and skilled Licensed Practical Nurse (LPN) specializing in assisted living care to join our team. 

Qualifications:

• Current BLS
• 1 year exp as an LPN/LVN
• Active Compact State license 

Job Summary: The primary purpose of this position is to provide direct, primary nursing care to ensure residents receive the necessary care and services to attain or maintain the highest practicable physical, mental, and psychosocial well being, in accordance with the resident's comprehensive assessment and care plan, and under the direction of a qualified professional nurse. The LPN participates in the planning, implementation, and evaluation of nursing care, within the objectives, standards, and policies of the department and through Community programs and procedures in compliance with applicable laws and regulations

Duties and Responsibilities:

• Collaborates with all disciplines in establishing the highest quality of care for residents.
• Provides direct and individualized nursing care to an assigned group of residents based on the application of scientific principles, under the supervision of a registered professional nurse.
• Shares responsibility for the planning, implementation, and evaluation of nursing care plans and nursing care.
• Receives, records, and carries out physicians' orders accurately and timely.
• Participates in team conferences, sharing knowledge, information, and ideas among team members.
• Collects data needed for the completion of Minimum Data Set.
• Identifies and reports changes in residents' status to physicians, responsible family members, and supervisory nursing staff.
• Prepares, administers, and records medications in accordance with the policy regarding the administration of medications by a licensed practical nurse.
